Sea View Hospital and Rehabilitation Center and Home is a 304-bed long term care facility located in Staten Island's lush Greenbelt. Serving the metropolitan area, it is easily accessible from all points by car and bus. Like many HHC facilities, Sea View has a long history. It opened its doors in 1913 as a sanitarium treating individuals suffering from the most feared disease of the time, tuberculosis. Today, the hospital is well-known for its long-term care brain injury unit, the first in New York State. Patients undergo intensive rehabilitation with the goals of restoring motor and cognitive functions and maximizing quality of life.

Job Description

Under the guidance and supervision of the Director, manages all Risk functions of the facility.

Duties & Responsibilities
1. Reviews electronically reported events on a daily basis.
2. Collaborates with Nursing and reviews completed accident/incident investigations to ensure proper records and supporting documentation.
3. Analyzes Risk Management data including occurrence reports and falls, infection control reports, mortality reports, medication error, etc., and to determine corrective actions.
4. Investigates complex clinical cases referred to the Department.
5. Assists in identifying problems that can result in a loss, risk of injury or harm to residents, visitors or employees. Identify trends and develop methods for risk reduction strategies, policy and/or organizational changes and their implementation from departmental reporting techniques.
6. Shares responsibility of Risk Management with the Director of Nursing and Medical Director in collaboration with other departments to design methods to achieve optimal patient safety initiatives via review and trending of patient safety data.
7. Schedules and facilitates Risk Management Committee meetings, compiles agenda and confidential minute preparation and appropriately circulates these to the Executive Director and all other members of the team prior to each monthly meeting.
8. Maintains Risk Management information database for internal and external use, in a safe, confidential manner, and to sequester charts, as needed.
9. Refers and interacts with NYCH+H offices for complex cases, i.e., Corporate Risk Management Council, Office of Legal Affairs, Office of Claims and Sedgewick Claims Management Services.
10. Responds to inquiries regarding various reports and individual professional liability matters.
11. Participates in developing new policies and procedures on safety, complaints, abuse or other matters that would improve patient care and quality of life, and reduce liability to NYCH+H. In preparation for NYS Department of Health surveys, gathers and processes necessary confidential reports and information for surveyors.
12. Assists with reporting incidents to the New York State Department of Health through Health Commerce System (HCS) as well as other regulatory and compliance agencies.
13. Coordinates investigations of cases by the Attorney General, Inspector General, NYSDOH and other regulatory agencies.
14. Reports to the Executive Director and Director of Quality Management, keeping E.D. informed of mandates and specific areas of concern that demand immediate attention regarding Risk Management. Immediately reports any unusual events or significant problems.
15. Active participation on Post-Acute Care service line-wide committees directed toward promoting quality of care and regulatory compliance; with availability to attend/cover other Committee meetings.
16. Attends Post-Acute Care service line-wide Risk Management Council Meetings.
17. Attends Risk associated NYC Health + Hospitals System Meetings.
18. Participates in internal/external Quality Assurance/Performance Improvement activities, programs and training, as required.
19. Completes Department of Health Reportable forms in preparation for the quarterly Post-Acute Care Board Report.
20. Serves as a Risk Management resource to all clinical and support departments by presenting relevant risk management issues for discussion and evaluation.
21. On-call responsibility to provide 24/7 coverage to inquiries from all nursing staff (outside normal business hours) on risk management and other concerns.
22. Assumes all functions of the Risk Management Office.
23. Performs related duties as required.

Minimum Qualifications

1. Master's degree from an accredited college or university in Risk Management, Health Administration, Business Administration or in a related health care specialization, and three (3) years of experience in risk management, control, and prevention, leading and implementing quality and risk management programs, managing and analyzing risk data, and performing other functions related to risk management in a hospital or a health care setting, one (1) year of which must have been in a responsible administrative, managerial or supervisory capacity; or

2. Bachelor's degree from an accredited college or university in fields, as described in "1" above, and four (4) years of risk management experience, as described in "1" above, two (2) years of which must have been in a responsible administrative, managerial or supervisory capacity; or

3. Bachelor's degree in the disciplines as described in "1" above; and two (2) years of clinical, patient care or equivalent experience in a hospital or health care setting, plus two (2) years of related experience, as listed in "1" above, one (1) year of which must have been in a responsible administrative, managerial or supervisory capacity; or

A satisfactory equivalent of education, training, and/or experience. Certified Professional in Healthcare Risk Management (CPHRM) or current license and registration to practice as a Professional Nurse, Nurse Practitioner or other similar clinical designation, may be Under the guidance and supervision of the Director, manages all Risk functions of the facility.
4. substituted for one (1) year of experience, however, all incumbents must have at least a bachelor's degree and one (1) year of responsible administrative, managerial or supervisory experience, as described in "1" above.
